志子田武四郎（吉沢亮）のもとへ、網走総合病院で救命医をしている親友・矢野悠太（高杉真宙）から電話があった。その際、悠太の様子がどこかおかしいことに気づいた武四郎は「何があったんだよ？」と心配するが、悠太は「なんでもない」と電話を切る。そんな悠太のもとに、7歳の少年・杉本淳之介（松野晃士）がトラックにはねられて緊急搬送されてきた。淳之介は、大腿骨骨折のほか、第4肋骨と第5肋骨が折れ、肺を損傷している可能性もあるほどの重症だった。外科医がオペ中だったため、悠太は、淳之介を設備の整った大きな病院へ搬送しようと決断するが、濃霧がひどくヘリコプターを飛ばせないとの知らせが入る。悠太は、このまま救急車で釧路に向かうよう指示するが、釧路までは山あいの道を進むことになるため、3時間はかかるといわれてしまい……。
